Fees
The cost of the program is $575, which includes an orientation session and 12 sessions. There is a sliding fee scale, based on family size and family income, for those who qualify for a reduction in the fee. Payments in 3 installments can be arranged. VISA, MasterCard, debit (when in-person) and E-transfers are accepted.
I agree to pay the fee, when due. I understand that I will not be allowed to attend if my account is in arrears. I also understand that failure to pay my fee may result in dismissal from the program prior to completion.
Your specific fee will be set when you meet with the coordinator at your intake.
For Agency Communication with Others
I give permission for the agency to exchange all relevant information concerning my attendance and participation with my referral source, the crown attorney’s office, court, police or the Halton Children’s Aid Society, if applicable.
Limits to Confidentiality
I understand that Thrive Counselling is required, by law, to disclose information about me under the following circumstances, with or without my written consent:
in response to a subpoena or search warrant
if a child under 16 years of age may be at risk of abuse
if they believe that I am, or someone else is, in danger of physical harm and they have information that might prevent it.
In keeping with professional standards, I understand that the counsellors will consult with each other, with their supervisor and other team members regarding my situation. All information discussed is strictly confidential.
Further Charges
If I become involved with Family or Criminal Court or other agencies while attending the program, I agree to notify the agency.

1. Probation & Parole I agree to an exchange of information between my Probation & Parole Officer named below, or another representative of that office, and Thrive Counselling.
The information exchanged may be about the offence for which I was charged, the terms of my probation/parole, and my attendance, participation and progress in the program. I understand that either party may inform the other of any breaches of my probation/parole order and any reports of further violence on my part.
