Program Registration for Thrive Services
Thank you for your interest in our services. In order to streamline our intake process, we request that you provide registration information and agree to our confidentiality policies and terms of service prior to completing the final steps of intake with our Intake Department.
After registration through our website, you will be contacted by our Intake or Program workers to complete the process. You can withdraw your interest or consent at any time. Sliding scale fees apply to the Individual, Couple and Family Counselling Program. Non-Halton clients are not eligible for the sliding scale subsidy and will be charged full fees.

Registration for Halton 360 hub, Use Counselling Registration form, note “hub” in Reason for referral or Primary concern
Halton 360 hub – Service Providers may complete referrals. Indicate Referring & your name in Consent section as needed.
If requesting a couple or family counselling, each person needs to complete the registration forms.

Many Thrive services are available through video-counselling. If you are able to use virtual services, please fill out this section of the form, even if you plan to attend in person.
Once we have received these forms, you will be contacted by Intake to finish your registration.
